INSURANCE

We more than happy to assist you in obtaining the insurance benefit that your insurance company allows toward oral surgery. We will bill your insurance as a courtesy to you with the insurance information you provide to us. It is very important that we have all the correct information before treatment is started. After the contract is signed and treatment has begun, no adjustments can be made to the contract for discounted plans. Insurance companies make no guarantee of payment. Each claim is processed at the time it is received based on your insurance status at the time of service.

You will be asked when treatment starts to sign a contract for the total treatment fee. You are responsible for the total fee and any insurance payment is deducted from that fee.

YOU ARE RESPONSIBLE FOR YOUR INSURANCE.

Your insurance company has a contract between you and your insurance or between you and your employer. We are not a party to that contract. We are unable to get involved in your insurance matters, such as denials, etc. Our contract with insurance companies is independent of your agreements. You will need to check to be sure your insurance is paying as they should, that the insurance has received the claim form, and that our office has all the correct information for billing.

Our practice is committed to providing the best treatment possible for our patients and we charge what is usual and customary for our area. You are responsible for payment in full regardless of any insurance company's arbitrary determination of usual and customary rates.
